Self-referral forms are not shared with doctors or nurses, so if you have a health concern or feel worried about your pregnancy for any reason, you should contact the community midwifery team directly

Once your appointment is made, you’ll receive an SMS (text message) inviting you to register to use UHCW’s patient portal, myhealth Coventry and Warwickshire

You’ll be able to use myhealth Coventry and Warwickshire throughout your pregnancy to:

• Access information about your upcoming appointments
• View test results and other clinical documents
• Communicate with your care team
• Access useful learning materials
